2024 ISCD Board 990 Questionnaire

ISCD is required to survey all voting board members on a yearly basis regarding activities during the last fiscal year. The questions were developed by our auditors and all responses will be forwarded to the auditors for use in IRS tax preparation. Please complete this form and submit so we can meet our reporting obligations.
